New Patient Paperwork

Our Services

At The Compass Clinic, each patient undergoes a thorough evaluation by the clinician 

including review of their history and medical records. Each patient's diagnostic work up 

is developed based on their specific history and exam results. A patient's initial 

assessment is usually completed in one to three visits. The Clinic encourages

collaboration with the Primary Care Provider and other specialists to provide top quality, 

individualized care.


Once a diagnosis is established, a treatment plan is formulated with the patient, their 

family, and Primary Care Provider.  The treatment plan considers symptomatic

medication options as well as appropriate non-pharmacological strategies to optimize

clinical outcome.  The Compass Clinic also works to help link families with the resources

they need while experiencing the emotional and physical changes after diagnosis. The 

local resources provide guidance and support for patients and their families and caregivers.

  • Medicare Part B (No Medicare HMO's) 
  • Blue Cross & Blue Shield - All products including Health Options HMO / no Medicare Advantage
  • United Healthcare - All products including Secure Horizons Medicare HMO
  • Humana - All PPO products and Humana Gold Plan/ No HMO 
  • AETNA - including HMO
  • Health Choice
  • Coventry - First Health / Mail Handlers Benefits
  • Patient is responsible to bring referral to office visit for any HMO plans.


We do not accept the following insurance companies:

  • Medicaid


Uninsured/Self Pay Rates:

  • First Visit: $250
  • Follow Up Visits: $175
